Clinic Registration

Welcome to the Clinic Registration section.

Before registering we wanted to give you an overview of how the process goes so you can register quickly and complete your own complimentary Lifestyle Health Risk Assessment to see the benefits of the LHRA for yourself.

1. Registration is Free.
2. Your first 2 Lifestyle Health Risk Assessments, 2 Spinal Health Assessments, and 90 Day Innate Lifestyle™ Plan are Free
3. Once you have completed the very brief registration form, our system will create a Clinic Account and a Patient Account for you. One of your complimentary LHRA's will automatically be assigned to your Patient Account.
4. The Clinic Account may take 1-2 business days to be approved. Once it has been approved, you will receive an email containing your Clinic Account information and login details. Your account will also have a second complimentary LHRA available in your inventory to assign to anyone you wish to. Unlimited patients can be added to the system and additional LHRA's, SHA's and 90 Day Innate Lifestyle™ Plans can purchased through the Clinic System.
5. Your Patient Account will be available immediately and you may complete the LHRA right away if you wish, or come back later and complete it then. We will will send you an email with your Patient Account details and login instructions.
